this email shows up from nowhere - justin@sample.com

The App was working fine all these days. Suddenly, i am seeing this justin@sample.com in the appsheet form.

This field should take the logged-in user and then show it here.

It has been working fine all these days correctly. It shows like this today morning.

Has anyone faced this issue before?

Have you used that email address before in your testing - e.g. changed the "preview app as" setting in the editor?  Could you be looking at the EDIT of an existing row instead a From view for the NEW row?

Thank you WillowMobileSys for your response. I got the answer from one of my colleges. This justin@sample.com is a default google appsheet email address and this issue seems to be a bug. We just need to logout and login. It will work fine after that.
